- All routine adjustment appointments and progress visits throughout active treatment
- Standard materials used during treatment
- Removal of braces at the end of your plan
- Bonded fixed retainers for both upper and lower arches, plus one set of removable retainers
- Retainer check appointments for up to two years after braces are removed

If a reimbursement check is sent directly to the subscriber rather than to the practice, that balance is handled the same way. Any amount not covered by insurance remains your responsibility.
